Hi all,
We are reviewing our Med Rec process in anticipation for a new Form available in our Health Authority for Med Rec, which will be generated using patient med profiles in the provincial, electroinc Drug Information Portal.
What is your program’s process for Medication Reconciliation?
-Frequency: How often do you do Med Rec?
-Responsibility: Who does Med Rec? How are med discrepancies resolved?
Also, how does your program keep track of medication changes?
-Ie How do patients report medication changes to the renal team? Do you use a medication tracking tool? Do you keep track of medications prescribed by the renal team differently than medications from other prescribers?

We recently revised our medication reconciliation policy at KHSC. We changed our frequency from Q3months to Q6weeks to align with physician’s Q6week blood work rounds. Our renal nurses in the hemodialysis unit complete medication reviews with the patient. Discrepancies are then documented for RPh, MD and NP to resolve and communicated verbally to the appropriate HCP if it is urgent.
All medications are listed on Nephrocare including the renal and non-renal ones.
We have seen great improvements on the accuracy of patient’s medication list since this change however nurse and patients are overwhelmed with the number of med reviews we do!
