Hello all. In light of the citrate collection tube shortage for INRs, are there any hemodialysis units out there that are actively switching patients on warfarin to a DOAC? Or perhaps already have a level of comfort with using DOACs in HD? If so, are there criteria that are used to switch eg, low bleed risk? Any info appreciated.

Hi Linda,
In Scarborough, many of our hemodialysis patients on warfarin for atrial fibrillation have switched over the apixaban a number of years ago. We occasionally see patients on DOAC s for DVT treatment as well.
If patients are on transplant lists they usually stay on warfarin.
