Hello all. A pharmacy generated MAR has been a mandate of Accreditation Canada. Works great for inpatients…not so much for an outpatient unit like hemodialysis. Are there any centres out there that use a pharmacy generated MAR for their hemodialysis unit medications? If so, do you have a 7 day MAR, 30day MAR? If not, how are doses of anything administered in the dialyis unit tracked? Looking forward to hearing, as the logistics of having a useful electonic MAR in our unit has been a tedious and frustrating endeavour.

We still use a MAR template which is filled out by hand by unit clerks and nurses for the medications administered in hemodialysis in most centres. St boniface hospital in winnipeg uses an electronic generated system so you could contact them if you wish.
We have a complete electronic list of all medications (in unit and outpt) that is updated whenever a patient has a medication change.
